What does it mean to "permanently lose stealth"?

Simply remove the stealth token. It does not prevent the minion from gaining stealth later.

Once the Master Tracker leaves play, does a future enemy that arrives with Stealth still have stealth?

Yes. But enemies that already lost Stealth won't regain it.
